A formal written authorization for high‑risk, non‑routine work — to identify hazards, set controls, assign duties, and make sure work is done safely, legally, and with full planning.

 

 

 

✅ THE 11‑STEP PTW PROCESS

 

1. Identify Work → Define scope and risks

2. Risk Assessment (JSA/JHA) → Map hazards and controls

3. Select Permit Type → Hot work, confined space, excavation, etc.

4. Prepare Permit → Complete all details and attach supporting docs

5. Review & Approve → Signed by an authorized competent person

6. Pre‑Work Talk → Brief team on hazards and rules

7. Set Controls → LOTO, barricades, ventilation, PPE, gas testing

8. Execute Work → Follow permit terms strictly

9. Monitor → Supervise; STOP WORK if conditions change

10. Clean & Close → Secure area, remove tools

11. Handover & Record → Final sign‑off, file lessons learned
