1. Basic Information
• Record date, time, exact location, persons involved, and reporting officer details.
2. Incident Description
• Describe what happened, where/when, who responded, and what systems/equipment were affected.
• Example: Smoke detected at AHU panel, technicians isolated power supply.
3. Root Cause Analysis (RCA)
• Identify causes such as equipment failure, human error, lack of preventive maintenance, or design flaws.
4. Impact Assessment
• Assess damage to equipment/assets, injury/health risks, and financial implications.
5. Corrective & Preventive Actions (CAPA)
• Take corrective steps: area restoration, photos, logs, alarm records, screenshots, thermal scans, etc.
