Incident & Near Miss Reporting

 Incident & Near Miss Reporting

:

In any workplace, incidents and near misses are important warning signs that should never be ignored. Proper reporting helps organizations identify hazards, analyze root causes, and implement corrective actions before a serious accident occurs.

A well-written Incident / Near Miss Report should always include:

✅Basic incident information (location, date, time, and personnel involved)

✅A clear and factual description of what happened

✅Identification of root causes such as unsafe acts, unsafe conditions, or equipment failure

✅Proper documentation and approvals from the safety team and management

Accurate reporting not only improves safety performance but also strengthens the overall safety culture within the organization.

Remember:

Today's near miss can become tomorrow's serious incident if it is not reported and investigated properly. Let's encourage everyone on site to report incidents and near misses honestly and promptly to prevent future accidents.
