Role & Responsibility:
Identify High-Risk Tasks: Determine if a task (hot work, confined space, electrical, height) requires a permit.
Risk Assessment: Evaluate hazards and identify necessary precautions.
Authorization: Review, sign, and issue the permit, validating that the worksite is safe to start.
Energy Isolation: Verify that all hazardous energy sources are properly isolated (Lockout/Tagout).
Site Inspection: Conduct a joint site inspection with the receiver.
Close Out: Review, close the permit upon completion, and ensure the area is safe.
2. Permit to Work Receiver (The "Doer")
The Receiver is the technician, contractor, or supervisor in charge of executing the high-risk work.
Role & Responsibility:
Understand Hazards: Read the permit, ask questions, and understand all risks and safety rules.
Site Setup: Ensure the area matches the permit requirements (barricades, ventilation, safety equipment).
Crew Briefing: Inform the work team about the hazards and safety precautions required.
Safety Compliance: Ensure the team follows all safety procedures (wearing PPE, using calibrated gas detectors).
Suspend Work: Stop the work and notify the issuer immediately if conditions change or if an emergency occurs.
Safe Completion: Clean up the site and return the permit to the issuer after job completion.
