For every major incident there are about 600 near miss incidents prior to it. Reporting & investigation of these incidents can help investigation & preventing a major accident in our jobs.
 

 
A Near miss is an incident, which resulted in no injury or illness and / or damage (loss) to property but has the potential to cause damage to people, asset or environment.


Near Miss Reporting

WHO         -           Any Employee can report a near miss.

WHAT       -           An incident which does not result in any damage

                              but had the potential to cause damage to people,

asset or environment.

WHEN       -           Immediately on spotting a near miss incident.

HOW         -           Inform your immediate supervisor and HSE Engineer.

WHERE    -           Fill the near miss report form available at site or report to safety section.

WHY         -           Reporting a near miss helps in investigating and
                               correcting the situation. Also reporting a near miss may win you appreciation and gift.
