Human Error Analysis (HEA) is an example of a structured way of identifying potential human failures (errors and rule violations) before they happen. It uses a set of guide words that can be used to help identify the sorts of errors and rule violations that could occur while a task is being performed.
Whenever an error event occurs, corrective and preventive actions (CAPA) must be developed to make an organization's systems stronger. A company will create systems to detect similar future errors or events, which will serve as the alarm that triggers corrective action and recovery.
There are three types of active human error as slips/lapses, mistakes, and violations, all of which are self-explanatory.
There are three types of errors that are classified based on the source they arise from; They are:
1. Gross Errors.
2. Random Errors.
3. Systematic Errors.
Human errors are not stopped, but they are detected after they occur.
The most common methods to detect human error are:
1. Inspect and Check: Test samples against a standard for non-conformances.
2. Work Process Audit: Inspect a work process and get information to better control the process (a feedback loop)

To Calculate Human Error Probability, Start at top with probability of correct/incorrect action.
Next act is probability of given the last action.
These are conditional probabilities - They are not independent.
Sum of partial error probabilities at bottom is overall error probability.
