🚨 INCIDENT REPORT | 5W & 1H APPROACH WITH HSE Trainer 🚨
🔥 One incident can become a powerful safety lesson — if we investigate, learn, and act.
This incident involved a fire in an electrical material storage area, highlighting the importance of proper battery charging, electrical safety, fire prevention, housekeeping, emergency preparedness, and effective incident investigation.
📋 WHAT HAPPENED?
A fire occurred in the electrical material storage area. The preliminary investigation indicated suspected battery overheating associated with charging/storage conditions. The root cause investigation should continue before any final cause is confirmed.
🔎 WHY IS ROOT CAUSE INVESTIGATION IMPORTANT?
A good incident investigation should not simply blame a worker or identify “carelessness” as the cause. The objective is to identify the underlying/systemic causes and implement corrective actions that prevent recurrence. OSHA specifically emphasizes investigating root causes rather than assigning fault.
⚡ KEY BATTERY & FIRE SAFETY CONTROLS
✅ Charge batteries only in a designated charging area
✅ Provide adequate ventilation during battery charging
✅ Keep combustible materials away from charging and electrical areas
✅ Control smoking, flames, sparks and other ignition sources
✅ Inspect batteries, chargers, cables and electrical equipment regularly
✅ Remove damaged, leaking, swollen, overheated or otherwise defective batteries from service according to the applicable procedure
✅ Follow the manufacturer’s charging and maintenance instructions
✅ Keep suitable fire protection readily accessible
✅ Maintain clear access to emergency exits and escape routes
✅ Ensure workers are trained in emergency response and battery-related hazards
OSHA requirements and guidance specifically address designated battery-charging areas, ventilation, fire protection and controls around ignition sources.
🧠 5W & 1H — ASK THE RIGHT QUESTIONS
🔴 WHAT? — What happened?
📍 WHERE? — Where did it happen?
🕐 WHEN? — When did it happen?
👷 WHO? — Who was involved or exposed?
❓ WHY? — Why did it happen?
⚙️ HOW? — How did the incident develop and spread?
🛠️ CORRECTIVE ACTIONS SHOULD ADDRESS:
• Equipment condition
• Charging practices
• Storage arrangements
• Housekeeping
• Electrical inspection
• Fire prevention measures
• Training and competency
• Procedures and supervision
• Emergency preparedness
• Root/system causes
📢 SAFETY LESSON:
Do not wait for an incident to teach us a lesson. Identify hazards early, report unsafe conditions, take corrective action, and prevent recurrence.
🛡️ Remember:
REPORT • REVIEW • LEARN • PREVENT
👷♂️ Safety is not just a rule — it is a responsibility shared by everyone.
