A Permit to Work is not simply a form or signature. It is a formal work-control and authorization system designed to ensure that hazardous or non-routine work is properly planned, risks are assessed, precautions are established, and the people involved understand the job conditions before work starts. ✅

🔹 WHY IS PTW IMPORTANT?

A properly implemented PTW system helps to:

✅ Identify hazards before work begins

✅ Assess risks and establish effective controls

✅ Define the exact scope, location and duration of work

✅ Confirm required isolations and LOTO

✅ Ensure workers are trained and authorized

✅ Coordinate simultaneous operations (SIMOPS)

✅ Establish emergency and rescue arrangements

✅ Improve communication between operations, maintenance, contractors and HSE

✅ Prevent unauthorized or uncontrolled work

✅ Ensure proper suspension, handover and close-out of the job

🔵 Permit Issuer / Authorizing Person

• Reviews the work scope

• Verifies hazards and risk controls

• Confirms required isolations

• Checks site conditions

• Ensures precautions are implemented

• Authorizes and signs the permit

• Communicates permit conditions

🟠 Permit Receiver / Performing Authority

• Understands the scope and permit conditions

• Conducts/participates in toolbox talk

• Ensures workers understand the hazards and controls

• Follows all specified precautions

• Stops work when conditions become unsafe or change

• Reports deviations and incidents immediately

• Ensures the permit is properly returned/closed

🔄 BASIC PTW LIFECYCLE

1️⃣ Work Request – Clearly define what work is required.

2️⃣ Risk Assessment – Identify hazards and determine controls.

3️⃣ Isolation & Preparation – Apply required LOTO, depressurization, draining, gas testing, barricading, etc., as applicable.

4️⃣ Permit Issue – Verify controls and authorize the work.

5️⃣ Toolbox Talk – Brief everyone on hazards, controls and emergency arrangements.

6️⃣ Work Execution & Monitoring – Perform work within permit conditions.

7️⃣ Suspension/Extension – Reassess and reauthorize when required.

8️⃣ Close-Out – Confirm work completion, restore systems safely and close the permit according to site procedure.

🔥 EXAMPLES OF WORK THAT MAY REQUIRE PTW

• Hot Work – welding, cutting, grinding

• Confined Space Entry

• Electrical work and isolation/LOTO

• Line breaking / opening process equipment

• Excavation where covered by site PTW requirements

• Lifting operations where controlled through the site's PTW system

• Work on hazardous process equipment

• Other high-risk or non-routine activities as defined by the site procedure

⚠️ IMPORTANT: PTW requirements depend on the applicable OSHA standard, industry, site procedure and type of work. PTW should not be treated as a substitute for risk assessment, isolation, engineering controls or competent supervision.

🛑 STOP WORK IF:

❌ Job conditions change

❌ Gas-test results become unsafe

❌ Required isolation is not verified

❌ Required PPE/control measures are missing

❌ Weather or site conditions create additional risk

❌ The actual work no longer matches the permit scope

📋 BEFORE STARTING THE JOB – ASK:

☑️ Is the scope clearly defined?

☑️ Have all hazards been identified?

☑️ Are controls actually in place?

☑️ Are required isolations verified?

☑️ Is gas testing required?

☑️ Are workers competent and authorized?

☑️ Has the toolbox talk been completed?

☑️ Are emergency arrangements available?

☑️ Are SIMOPS identified and controlled?

☑️ Is the permit valid for the current conditions?

⚠️ GOLDEN RULE

NO VALID PERMIT + NO REQUIRED CONTROLS = NO WORK.
